CasualTranscriber is a transcription helper application for Mac, which can handle various formats of audio/video files that QuickTime can open. The default text file format is Rich Text Format (RTF), and you will import a media file to control it and transcribe the text. You can also open/save MS word documents (.doc) or plain text files (.txt).
