It can clean up all unnecessary files left behind by other programs, either as a consequence of a previous uninstall process or for any other reason. Also, it can help you remove unwanted auto-run programs to speed up the startup process. Finally, you can also use this tool to move all your programs to a new drive or computer, simply by backing them all in a single file and restoring it in the new disk. This may not work with certain programs though, such as the Microsoft Office suite of tools, which seems to be very picky when it comes to re-installs processes.
All in all, Total Uninstall is certainly a useful addition to your desktop. It shares some of its functionality with the already existing Windows uninstall tool, but it goes way beyond what your SO is willing to do for you when uninstalling unwanted apps and programs.
